Signature: eSign (you can type out your full legal name if sending through our proper support channel).The statement you sign at the end of your notice is an accuracy statement.

Get clarification if the report says anything you don't understand.
Use reasonable efforts to prevent the posting of the same content more than once.
Keep in-house records of notices to ensure you’re always in compliance and deal with repetitive issues.
